I, the undersigned, hereby authorize Young's Collision Center and its employees to repair my vehicle, and to drive my vehicle for the purpose of testing or inspection.
I understand that an express mechanic's lien is acknowledged on the vehicle to secure the amount owed for repairs made to it.
Young's Collision Center will not be held responsible for theft of the vehicle or for articles left in the vehicle.
Young's Collision Center is unable to release any vehicle without full payment. We accept insurance checks endorsed over to Young's Collision Center, personal checks, and cash.
I authorize the insurance company to pay Young's Collision Center directly for all original and supplemental repairs, and I authorize Young's Collision Center to endorse my name on any insurance checks received as payment for repairs.
